The submission of the Homelessness Prevention Program (HPP) Follow-Up Report by the ETHS and HPP Local Administering Agencies (LAAs) upon submission HPP grants application. The purpose of the HPP Follow-Up Report is to track the status of individuals HPP program 3 months after the HPP eviction prevention grants were issued collect and compile appropriate follow-up data from all local HPP grant recipient combined report to the Office of Transitional Services. The HPP Follow-Up Transitional Services by the 15th day of the month following the "follow-up to: Rick Urps Maryland Department of Human Resouces 311 W. Saratoga Street, Room 247 Baltimore, MD 21201 The report can also be sent via e-mail at Rurps@dhr.state.md.us or faxed to Rick you have any questions about the report, please call Rick on 410 767-7219.
