How to Submit a Request

When possible, please submit your request 1 month before the print or event deadline. Within your request, please include the following information:

  • Your name, mailing address, email address and phone number
  • The location, date and time the award is scheduled for presentation
  • Name and address of recipient/event (ex: Eagle Scout, Century Club, Adoption Awareness Month)
  • 3-4 short descriptive sentences to be included in the text of the award
  • Other special requests or details
