    1. I have read and agree to abide by the Teen Court Dress, Conduct and Behavior Code. I understand that I will not be permitted to participate in court if my attire does not comply with the dress code.
    2. I understand that Teen Court volunteers are responsible for their own transportation.
    3. I understand that I must keep the details of Teen Court confidential.
    4. I understand that Teen Court starts promptly at 4:30 p.m. and may run as late as 7:30 p.m.. and I am required to attend the whole session, unless excused by Teen Court staff.
    5. I understand that this signed application can be used to participate in all future activities and sessions of Teen Court.
    6. I will keep Teen Court advised of any change in school or contact information.

    "I consent to the photographing of my child during sessions of the Harford County Teen Court and its release to be used for education and research by the Harford County Government of Harford County Sheriff's Office.  I release the Harford County Government or Harford County Sheriff's Office and the District Court of Maryland and each and all persons involved from any liability connected with the taking, recording for publication of said court session.  I waive all rights I may have to any claims or payments or royalties in connection with the use of these materials regardless of the purpose of such broadcasting.  I have fully informed myself of this consent, waiver of liability, and release before signing it."
